    General Description:

    Chronic gastritis is the inflammatory lesion of the gastric mucosa caused by a variety of factors. Its clinical manifestations include epigastric discomfort or pain, anorexia, nausea, vomiting, belching, diarrhea, etc. Chronic gastritis has a high incidence with more than 20% of the adults affected and the incidence increases with age. According to the morphological change under the gastrofiberscope, this disease is classified into chronic superficial gastritis and chronic atrophic gastritis. The superficial layer of the gastric membrane is mainly affected in chronic superficial gastritis while chronic atrophic gastritis is characterized by the atrophy of the connatural glands of the gastric membrane, often accompanied with intestinal metaplasia and inflammatory reaction.

    Chronic gastritis belongs to the categories of "wei tong"(gastric pain), "pi man" (feeling of fullness), "ou tu" (vomiting), "cao za" (gastric discomfort) and "tun suan" (acid regurgitation) in TCM. It is believed that chronic gastritis is due to improper diet, irregular intake of food, or overeating of cold and raw food, irritating food, cigarette smoking and indulge in alcohol which damdage the spleen and stomach, causing undescending of stomach qi and qi stagnation; it may also be caused by emotional disturbance which results in stagnant liver qi attacking the stomach, or stagnant liver qi turning into fire which damages stomach yin; or longterm disorders causing qi and blood stagnation in the stomach channel.

    Essentials for Diagnosis:

    (1) The main clinical manifestations include vague pain, burning pain, dull pain or distention in the epigastrium, especially after meals, frequently accompanied with belching, nausea, vomiting and anorexia. In general, there is no obvious physical sign, but slight tenderness appears sometimes.

    (2) The secretion of the gastric juice is generally normal in chronic superficial gastritis, but its acidity is sometimes lower or higher. The secretion of gastric juice in a patient with atrophic gastritis is generally less than that in normal patients. The shortage of gastric juice depends on the location and scope of the atrophic area.

    (3) The gastroscopy is the most reliable method to diagnose chronic gastritis. The pathological changes of chronic superficial gastritis is commonly seen in the gastric antrum manifested as congestion, edema, or bleeding and erosion of the mucous membrane. Chronic atrophic gastritis is mainly marked by glandular atrophy in the gastric mucosa or epithelial hyperplasia, the erosion and bleeding of the mucous membrane may also be present.

    (4) The examination of the pylorus helical bacillus is generally positive, which may be done through gastric juice culture and smear, or urease test.

    Syndrome Differentiation and Treatment:

    The differentiation of chronic gastritis is mainly to identify different syndromes in light of TCM theory. Is the case caused by qi stagnation or by blood stagnation? Does the case pertain to cold syndrome or to heat syndrome, to deficiency syndrome or to excess syndrome? Generally, the case with a long course and manifested as cold pain, preference for hot food, vomiting of watery fluid belongs to cold syndrome. The case with a short

    course and manifested as burning pain, preference for cold food, vomiting of acid fluid belongs to heat syndrome. Deficiency syndrome has a long course with preference for pressure at the painful area, pain aggravated by hunger and alleviated by intake of food. Excess syndrome has a short course with pain aggravated by pressure and intake of food, alleviated by hunger. The case with a short course and manifested as paroxysmal painand distention is caused by qi stagnation. The case with a long course and manifested as continuous, fixed and pricking pain is caused by blood stagnation. The treatment should be based on the principle of regulating the flow of qi, normalizing the function of the stomach and relieving pain. For excess syndrome, cold accumulation, qi stagnation, stomach heat, blood stagnation should be identified and treated respectively by dispersing cold, relieving pain, soothing the liver, clearing liver and stomach heat and removing blood stasis in the channels. For deficiency syndrome, yang deficiency and yin deficiency should be identified and treated by warming the stomach and strengthening middle energizer or nourishing stomach yin.

    A. Pathogenic Cold Attacking the Stomach

    Chief Manifestations: Sudden onset of gastric pain which is aggravated by pressure and alleviated by hot compression, preference for warmth and aversion to cold, absence of thirst, preference for hot drinks, white tongue coating, wiry and tense pulse.

    Therapeutic Methods: To disperse cold, warm the stomach, regulate the flow of qi and relieve pain.

    Prescription: Radix Notogniseng, Radix Paeoniae Alba, Rhizoma Cyperi, Radix Glycyrrhizae, etc.

    Chinese Patent Drugs: Stimache & Ulcer Pills

    This pills not only can help you relieving gastric hyperacidity and gastralgia, removing blood stasis and arresting bleeding but also to disperse cold, warm the stomach regulate the flow of qi and relieve pain.

    B. Damp Heat in the Spleen and Stomach
