General Description

Gout is a kind of disease caused by prolonged purine metabolic disturbance, clinically characterized by hyperuricemia, repeated attack of acute arthritis, gouty tophus sedimentation, chronic arthritis with deformity of joints, pathogenic change of renal parenchyma and formation of tophus. The disease is grouped into two categories by different causes of increased uric acid in blood, i.e., primary and secondary. Primary gout is caused by congenital purine metabolic disturbance and secondary gout is caused by hyperuricemia due to the increase of uricopoiesis or hypoeccrisia of uric acid, which is induced by other diseases or drugs. The disease may occur in people at any age, mostly at the age between 30 - 40. Among the patients, 95% are male and usually they have a family history of gout.

Because gout is clinically manifested as recurrence of redness, swelling, a burning sensation, pain, and limited flexibility of joints, it is included in the category of bisyndrome in TCM. As for the pathogenesis, this disease is externally caused by blockage of the meridians following invasion of the body by exogenous pathogenic wind, cold and damp heat; and internally caused by deficient healthy qi, overstrain, or exposure to wind while sweating and the weakened weiqi failing to protect the body from the exogenous pathogenic factors, giving rise to blockage of the meridians and stagnation of qi and blood. In a prolonged case, there may appear blood stasis and phlegm stagnation with the involvement from the meridians to the viscera, then visceral hisyndrome occurs.

Essentials for Diagnosis

(1) At the initial stage, it is symptomless with only the increase of uric acid in blood. At the acute stage, its typical manifestation is sudden onset of arthralgia and most of the cases would suddenly be woken up by sharp arthralgia when sleeping. The affected parts are mostly the first metatarsophalangeal joint or other metatarsophalangeal joints with localized redness, swelling, a burning sensation, pain and limited flexibility. When the greater joint is involved, there may be some exudate. In addition, such symptoms as chills, fatigue, poor appetite and headache may appear. However, the above mentioned symptoms would be relieved in 1-2 weeks.

(2) At the chronic stage, there may be gouty tophus, deformity and rigidity of joints, and renal lesions.

(3) Laboratory examination shows uric acid in blood increasing to over 420 umol/L. In roentgenogram, a round or irregular transparent defective area can be found in the bone substance of the joint adjacent to osteochondral edge, which is characteristic of gout in Xray examination.

Syndrome Differentiation and Treatment

The pathological factors of this disease are chiefly dampness, phlegm and blood stasis, and therefore removing dampness, resolving phlegm and eliminating blood stasis are the basic treatment principles. However, dampness is classified into cold dampness and damp heat and different treatments should be adopted according to syndrome differentiation. At the later stage, spleen yang and kidney yang are usually impaired. Thus it is advisable to eliminate pathogenic factors and support healthy qi as well.

A.Obstruction of Damp Heat in the Meridians

Chief Manifestations: Redness, swelling, a burning sensation and pain of the joints which is alleviated by cold, tenderness, or fever, constipation, dark yellow urine, yellow and greasy tongue coating, and wiry and rapid or smooth and rapid pulse.

Therapeutic Methods: To clear away heat, eliminate dampness, and remove obstruction in the collaterals.
