    General Description

    Depressive syndrome is due to emotional upsets and stagnation of qi, clinically manifested as mental depression, restlessness, feeling of fullness and oppression over the chest, distending pain over the hypochondrium or irritability and

    susceptibility to cry, or feeling of a foreign body in the throat.

    The cause of depressive syndrome is emotional upsets. Pathogenically, if the liver fails to control the dispersion, the stagnant liver qi attacks the spleen, which impairs the spleen's function of transportation and transformation and the heart is deprived of nourishment. Thus, disorders of the viscera, yin, yang, qi and blood appear.

    At the initial stage, depressive syndrome is attributed to the excess type. It is usually caused by stagnation of qi, complicated by blood stagnation, fire transformation, phlegm accumulation and food retention. But prolonged cases will change from the excess type into the deficiency type, and there may appear different pathological changes of the heart, spleen, liver or kidneys, as a result of different conditions of the involved zang-fu organs and of the consumption of qi, blood, yin or yang.

    According to the clinical manifestations and its pathogensis, depressive syndrome is mainly seen in neurasthenia, hysteria and anxiety in western medicine, and also in perimenopausal syndrome and reactive psychosis.

    Essentials for Diagnosis
    (1) The major symptoms are mental depression, restlessness, distending pain over the chest and hypochondrium or irritability and susceptibility to cry, or feeling of a foreign body in the throat, mostly seen in young and middle-aged women.
    (2) The patient usually has a history of emotional upsets, such as melancholy, anxiety, sorrow, terror and resentment. Its recurrence is closely related with the emotional factors.

    (3) Examinations of various systems and laboratory examinations are normal. Organic diseases should be excluded.

    Syndrome Differentiation and Treatment

    At the initial stage, depressive syndrome is mostly of the excess type, and then the prolonged case changes from the excess to the deficiency type or the deficiency- excess type. The principle of the treatment is to regulate qi circulation. The case of the excess type should be treated by soothing liver qi and alleviating mental depression, and assisted by promoting blood circulation, resolving phlegm, promoting diuresis, clearing away heat and promoting digestion; the case of the deficiency type should be treated by nourishing the heart, tranquilizing the mind and nourishing the heart and kidneys.

    1. Stagnation of Liver Qi

    Chief Manifestations:Mental depression, restlessness, feeling of fullness and oppression over the chest, distending and wandering pain in the hypochondrium, abdominal flatulence, eructation, loss of appetite, irregular bowel movements, thin and greasy tongue coating, and wiry pulse.

    Therapeutic Methods:To soothe liver qi, alleviate mental depression and regulate middle energizer qi.

    Prescription: Chuan xiong, Ginseng, Rhizoma Rehmanniae Praeparata, Rhizoma Alismatis, Poria, etc.
