The disease is usually attributive to excessive sexual activities and masturbation or frequent seminal emission that characterizes deficiency of essence. Essentials for Diagnosis (1) The disease is characterized by inability to have an erection of penis during sexual intercourse in males befare climacteric. (2) It can be attributive to excessive sexual activities, prolonged illness, weak constitution, frequent masturbation in youngsters, accompanied by lassitude, weak loins and knees, aversion to cold, cold limbs, or difficulty in urination with dribbling urine.
(4) Distinguish impotence from premature ejaculation. Premature ejaculation refers to early ejaculation of semen at the beginning of sexual intercourse and inability to carry on sexual intercourse because of flaccidity of the penis immediately after the ejaculation. However, impotence means the inability to achieve a satisfactory penile erection. They present marked differences. Nonetheless, if premature ejaculation persists, impotance may ensue. Syndrome Differentiation and Treatment Differentiation of the disease primarily concerns whether or not impotence pertains to deficiency syndrome or excess one, whether or not there exists the hyperactive fire. Impotence attributive to sex excess, anxiety, depression and fright is mainly indicative of deficiency of the spleen and kidneys, decline of fire of the life gate, thus pertaining to deficiency syndrome. Impotence due to the transformation of stagnant liver qi into fire, downward movement of damp heat and flaccidity of penis pertains to excess syndrome. Impotence with pale complexion, aversion to cold, cold limbs, pale tongue with white coating, deep and thready pulse is indicative of the nonexistence of fire; while the one with irritability, dark yellow urine, greasy and yellowish tongue coating, softsuperficial, rapid or wiry, rapid pulse is indicative of the hyperactive fire. To treat impotence, tonifying method should be employed for deficiency syndrome and purgative method for excess syndrome; heat clearing method for hyperactive fire and warming method for nonexistence of fire.
