5. Hordeolum

Case 1

Name: Mi.

Sex: Female.

Age: 18 years.

0ccupation: A college-student.

Chief Complaint: Swelling pain of dextral upper eyelid for half a month.

History of Present Illness: Half a month ago, the patient got a swelling-pain in her dextral upper eyelid without any causes. At the beginning, it was limited to open the dextral eye while without  swelling pain, some  times  later, she  got  a  severe swelling pain in the dextral upper eyelid so that she couldn't open the dextral eye at all. Then she went to the local policlinic and was treated by using "Guttae Kanamycini Sulfatis, eye drops of anti-inflammatory" and "chlorotetracycoline eye ointment. " After 6  days  of  the  above  treatment, she  got  no  effect, furthermore there was a severe swelling region on the dextral upper eyelid, in which there was a triticeous node. If touched upon, there was sharp tenderness. Then she came to our hospital for treatment on May 29,1996.

Physical Examination:  The patient was conscious with a normal general condition. The lateral part of the dextral upper eyelid was red and swollen, there was a triticeous node in the affected  part  with sharp  tenderness.  The  tarsus  and  the conjunctiva were severely congestive. The pulse was floating. The tongue was red with a thin layer of white fur. There wasn' t other abnormal sign.

Diagnosis : Hordeolum.

Differentiation: It's superficies syndrome of TaiYang caused by attack of exogenous wind-heat, by which the blood stasis and stagnation of qi was caused in the affected region. Then the patient got a swelling-pain in her dextral upper eyelid.

Therapeutic Principle:  Expelling exogenous wind heat and relieving superficies syndrome of TaiYang. Then the trans-mission point of TaiYang small intestine channel should be stimulated to activate flowing of meridian qi and expell the exgenous wind heat.

Treatment : She was told to take a handful of moxa granules and rub them into peanut like moxa cones, and then she should put a moxa cone onto the point of  Houxi (SI3)  to  have direct moxibustion, 10 pieces of moxa cones were used each time, 3 times a day.

Effect Observation: After 1 day' s treatment, the swelling-pain was relieved greatly, after 3 days' treatment, the swelling-pain and the hordeolum disappeared. After 1 week's follow up, the illness didn't recur. Then it's ending.

 

 

Case 2

Name: Zhou.

Sex: Female.

Age: 50 years.

Occupation: A worker.

Chief Complaint: Redness and swelling of sinister upper eyelid for 1 week.

History of Present Illness: One week ago, the median part of the patient's sinister upper eyelid became red and swollen without any causes, accompanied with slight itching sensation. After she rubbed the affected part by herself, there appeared a red triticeous node in the affected part, so that it's difficult for her to open the sinister eye. Then she dropped some "eye drops of antiinflammatory" into the eye to relieve the syndrome, while there wasn' t obvious effect on her. Then she came to our hospital for treatment on June 16,1998.

Physical Examination: The patient was conscious with a normal general condition. The pulse was taut and thready. The tongue was red with a dry and rough layer of white fur. The median part of the sinister upper eyelid was red and swollen, where there was a tensile triticeous node with tenderness. There wasn't other abnormal sign.

Diagnosis : Hordeolum.

Differentiation: It's superficies syndrome of TaiYang caused by upward attack of exogenous wind-heat, which caused the blood stasis and stagnation of qi. Then the patient got a hordeolum.

Therapeutic Principle: Clearing away heat evil and expelling pathogenic wind; cooling blood and removing blood stasis.

Recipe:

  ShengDi H uang, Dried Rehmannia Root 15 g

  MuDanPi,Tree Peony Bark 12g

  ZhiMu, Windweed Rhizome 12g

  ShengShiGao,Gypsum 30g (to be decocted earlier)

  HuangLian,Coptis Root 9g

  HuangQin,Scutellaria Root 12g

  DangShen, Red Sage Root 18g

  XiaKuCao, Prunella Spike 18g

  ZhuYe,Bamboo Leaf 12g

  ChanYi,Cicada Slough 9g

  BaiJiangCan,Batryticated Silkworm 9g

  BoHe,Peppermint Herb 9g (to be decocted later)

  JueMingZi,Cassia Seed 25g

  LongDanCao,Gentian Root 10g

  ShengGanCao,Raw I.iquorice Root 10g

    All the above drugs were to be decocted in water for oral taking. 3 doses were prescribed. In the same period, the point of Houxi(SI3) was used to have a moxibustion, it's beneficial to relieving the TaiYang superficies syndrome.

Effect Observation: After 3 days, the red and swollen region became normal and the hordeolum disappeared. After 1 week's follow up, the sickness didn't recur. Then it's ending.
