Syndrome of Obstruction of Collaterals by Cold and Dampness
Main Symptoms In the infected limbs, there are such symptoms as preference of warmth and aversion to cold, pale skin, ice cold, numbness and pain aggravated by cold, difficult walking, intermittent claudication, accompanied by cold sensation in the extremities of the four limbs, aversion to cold and contraction, slightly red tongue, white and greasy coating, deep and thready pulse.
Therapeutic Methods Warm meridians, disperse cold, activate blood and dredge collaterals.
Formula Modified "Huangqi Guizhi Wu Wu Decoction" with baked Huangqi (Radix Astragali seu Hedysari) 15 g, Guizhi ( Ramulus Cinnamomi ) 10 g, Chishaoyao ( Radix Paeoniae Rubra ) 6 g, Xixin ( Herba Asari) 4g, Danggui (Radix Angelicae Sinensis) 8g, Ganjiang (Rhizoma Zingiberis) 3 g, Chuanniuxi (Radix Cyathulae) 12 g, and baked Gancao (Radix Glycyrrhizae) 5 g.
Modification For severe aversion to cold and cold sensation in the limbs, add prepared Fuzi (Radix Aconiti Praeparata) 10 g and Rougui ( Cortex Cinnamomi )
(Decoct later) 3 g. For obvious pricking pain, add prepared Ruxiang (Resina Olibani ) 3 g, prepared Moyao (Myrrha) 3 g and processed Chuanshanjia ( Squama Manitis) 10g. For muscular spasm, add Mugua (Fructus Chaenomelis) 12 g and fried Baishaoyao (Radix Paeoniae Alba) 15 g.
Stasis of Blood Vessels
Main Symptoms Aggravated soreness and distension in the local region, heavy and weak steps, obvious motor impairment, dark red color turned from pale color
in the skin of the infected region, more obvious dark red color and aggravated pain in dropping the limb, and even insomnia, dark red tongue, or with purple spots, thready and wiry and hesitant pulse.
Therapeutic Methods Activate blood, disperse stagnation, dredge collaterals and stop pain.
Formula Modified "Tao Hong Si Wu Decoction" with Taoren ( Semen Persicae ) 10 g, Honghua ( Flos Carthami ) 3 g, Danggui ( Radix Angelicae Sinensis ) 6 g, Chishaoyao (Radix Paeoniae Rubra) 10 g, Danshen ( Radix Salviae Miltiorrhizae) 15g, Mudanpi ( Cortex Moutan Radicis) 6 g, processed Chuanshanjia (Squama Manitis) 4 g, Dilong (Lumbricus) 1O g, prepared Ruxiang (Resina Olibani) 3 g, prepared Moyao (Myrrha) 3 g, and baked Gancao (Radix Glycyrrhizae) 5 g.
Modification For obvious aversion to cold in the infected region, add fried Guizhi (Ramulus Cinnamomi) 10 g and Xixin (Herba Asari) 3 g. For obvious heat manifestation, add Pugongying (Herba Taraxaci ) 15 g and Zihuadiding ( Herba Violae ) 15 g. For muscular spasm, add fried Baishaoyao (Radix Paeoniae Alba ) 12 g and Mugua (Fructus Chaenomelis) 10 g.
Syndrome of Excessive Dampness and HeatToxin
Main Symptoms Tumefaction in the infected region, purple dark color in the skin, progressive purple dark color, and rupture and decay, serious pain alleviated in the daytime and aggravated at night to cause insomnia, accompanied by fever, dry mouth, brown urine, red tongue, thin and yellow coating, wiry and rapid pulse.
Therapeutic Methods Clear away heat, remove dampness, dissolve toxin and disperse stagnation.
Formula Modified "Si Miao Yong An Decoction" with Jinyinhua ( Flos Lonicerae) 10 g, Xuanshen (Radix Scrophulariae) 15 g, Danggui (Radix Angelicae Sinensis) 6 g, Chishaoyao (Radix Paeoniae Rubra) 12 g, Mudanpi ( Cortex Moutan Radicis ) 10 g, Zicao ( Radix Arnebiae seu Lithospermi ) 12 g, Cheqianzi ( Semen Plantaginis ) ( Decoct in pack ) 15 g, raw Yiyiren (Semen Coicis) 20 g, and raw Gancao (Radix Glycyrrhizae) 5 g.
Modification For excessive heat toxin, add Huangbai (Cortex Phellodendri ) 10 g and Shanzhizi (Fructus Gardeniae) 10 g. For rupture with blood fluid, add Zihuadiding (Herba Violae) 15 g and Qiancao (Radix Rubiae) 12 g. For lingering exfoliation of decayed flesh, add processed Chuanshanjia ( Squama Manitis) 8 g and Zaojiaoci ( Spina Gleditsiae) 12 g.
Syndrome of Yin Impairment due to Heat-Toxin
Main Symptoms Dry skin in the infected region, exfoliation of hair, thickened and deformed nails, muscular atrophy, dry necrosis in the local region, accompanied by fever aggravated at dusk, aching joints, insomnia,slightly red tongue, scanty coating, thready and rapid pulse.
Therapeutic Methods Nourish yin, dissolve toxin, activate blood and dredge collaterals.
Formula Modified "Gu Bu Decoction" with raw Huangqi ( Radix Astragali seu Hedysari ) 15 g, Shihu (Herba Dendrobii ) 12 g, Jinyinhua ( Flos Lonicerae ) 6 g, Danggui ( Radix Angelicae Sinensis ) 5 g, Niuxi (Radix Achyranthis Bidentatae ) 10g, Juhua ( Flos Chrysanthemi ) 12 g, Pugongying ( Herba Taraxaci ) 15 g, and raw Gancao (Radix Glycyrrhizae) 5 g.
Modification For severe yin damage, add Tianmendong ( Radix Asparagi ) 12g, Maimendong ( Radix Ophiopogonis) 12 g and Tianhuafen (Radix Trichosanthis ) 15 g. For retention of stagnant blood, add Shensanqi (Radix Notoginseng) (Take after infused with water) 6 g, Sanleng (Rhizoma Sparganii) 6 g and Ezhu (Rhizoma Zedoariae) 6 g. For excessive residual toxin, add Zihuadiding (Herba Violae) 15 g and Lianqiao (Fructus Forsythiae) 12 g.
Other Therapies
Application Method
In the primary stage, it is advisable to use "Chong He Ointment" or "Hui Yang Yu Long Ointment" for external application.After rupture, it is advisable to mix with"Jiu Yi Pellet" and cover with "Hong You Ointment". After the decayed flesh is gone, it is advisable to use "Sheng Ji Powder" and cover with "Sheng Ji Ointment".
Fumigation and Irrigation Method
It is advisable to decoct Danggui (Radix Angelicae Sinensis) 15 g, Sangzhi ( Rmulus Mori ) 30 g, Weilingxian (Radix Clematidis) 15 g and Sumu (Lignum Sappan) 30 g with water to fumigate and wash the infected region, for 30 minutes each time, once a day.
Surgical Method
In accordance with different situations, it is advisable to choose debridement of necrotic tissue, resection and suture of necrotic tissue, amputation and dermatoplasty.

Cautions
