Syndrome differentiation and treatment
Exterior syndrome and interior syndrome should be distinguished from each other in the syndrome differentiation of rubella. Mild rubella with low fever, nasal obstruction with discharge, skin rashes distributing in the hair,itching skin, sparse rashes with red colour and even distribution and natural abatement of skin rashes within 2 to 3 days indicate that the pathogenic factors stagnate in the
exterior; high fever without abatement, dysphoria, thirst with the inclination to drink, dense rashes with bright red or deep purple colour reveal that excessive pathogenic heat has invaded the interior, belonging to severe syndrome in most cases.
The basic therapeutic principles of rubella include dispelling wind and clearing away heat. Clinically, for the patients with more excessive wind, the main therapeutic method is expelling wind with pungent-flavoured drugs;for the patients with excessive pathogenic heat, the therapeutic method is clearing away heat or accompanied by cooling blood and purging heat.
Stagnation of pathogenic factors in the Lung-dominated defensive system
Main symptoms and signs Fever after onset, no aversion to wind and cold, sneeze, nasal discharge,cough, thirst, normal state of spirit, poor appetite, small
and sparse skin rashes with red colour distributing all over, itching skin, gradual abatement of rashes within 2 to 3 days, lymphadenhypertrophy of retroauricular, cervical and occipitoposterior regions, red tongue with yellowish and thin fur, floating and rapid pulse.
Therapeutic methods Dispelling pathogenic wind and clearing away heat to promote eruption.
Prescription and drugs The prescription used is the modified Yinqiao Powder (Powder of Lonicera and Forsythia) composed of 10 g Jinyinhua (Flos Lonicerae),
6 g of Lianqiao ( Fructus Forsythiae ), 6 g of Zhuye ( Herba Lophantheri ), 10 g of Niubangzi ( Fructus Arctii), 3 g of Jiegeng ( Radix Platycocli ), 5 g of Bohe
(Herba Menthae) (to be decocted later) and 3 g of Gancao ( Radix Glycyrrhizae ) .
Modification For the patients with skin itching, 6 g of Chantui (Periostracum Cicadae) and 10 g of Baijili (Fructus Tribuli) are added; for the patients with cough
and thick sputum, 10 g of Zhebeibu (Bulbus Fritillariae Thunbergii) and 10 g of Nanshashen (Radix Glehniae) are included; for the patients with restless nocturnal sleep and dysphoria, 10 g of Gouteng (Bamulus Uncariae cure Uncis) (to be decocted later) and 10 g of Baijiangcan ( Bombyx Batryticatus ) are added.
Excessive pathogenic heat
Main symptoms and signs High fever without abatement, dysphoria, thirst with inclination to drink,poor appetite, dense skin rashes with red or deep red colour, lymphadenhypertrophy with obvious tenderness on retroauricular, cervical and occipitoposterior regions,constipation, dark and scanty urine, red tongue with yellowish fur, rapid and forceful pulse.
Therapoutic methods Clearing away heat to cool nutrient system, promoting eruption and removing toxin.
Prescription and drugs The prescription used is the modified Touzhen Liangjie Decoction (Eruption-promoting and Heat-removing Decoction) composed of 10 g of
Sangye ( Folium Mori ), 10 g of Juhua ( Flos Chrysanthemi), 10 g of Xianhecao (Herba Agrimoniae), 10 g of Lianqiao ( Fructus Forsythiae ), 10 g of Chishaoyao (Radix Paeoniae Rubra ) , 10 g of Zihuadiding ( Herba Violae), 15 g of Banlangen ( Radix Isatidis ), 10 g of Mudanpi (Cortex Moutan Radicis ) and 6 g of Gancao (Radix Glycyrrhizae ).
Modification For patients with high fever without abatement, 30 g of Shengshigao (Gypsum Fibrosum) (to be decocted first) and 30 g of Hanshuishi (Calcitum) (to be decocted first) are added; for the patients with thirst and inclination to drink, 10 g of Tianhuafen (Radix Trichosanthis ) and 10 g of Shihu (Herba Dendrobii) are included; for the patients with dense skin rashes with red or deep red colour, 10 g of Shengdihuang (Radix Rehmanniae) and 10 g of Zicao (Radix Arnebiae seu Lithospermi) are added;for the patients with constipation, 6 g of Dahuang (Radix et Rhizoma Rhei ) (to be decocted later) and 10 g of Zhishi (Fructus Aurantii) are included; for the patients with dysphoria, 3 g of Huanglian (Rhizoma Coptidis) and 10 g of Zhuye (Herba Lophantheri) are added.

Xiao'er Zicao Pill (Infantile Pill of Arnebia or kithosporm) The Pill, applicable to excessive pathogenic heat, is taken twice a day, one pill each time.
Infantile Eruption Pill The pill, applicable to excessive pathogenic heat, is taken twice a day, one pill each time.
Wuli Huichun Pill The pill, applicable to occasional fright with excessive pathogenic heat syndrome, is taken twice a day, 5 pills for children above the age of 3,and for children below 3 the dosage is accordingly reduced.
Simple and proved recipes
30 to 60 g of Lugen (Rhizoma Phragmitis) and 30 g of Zhuyexin (Tip of Herba Lophantheri) are decocted to be taken like tea. It is applicable to stagnation of
pathogenic factors in the defensive system.
10 g of Jinyinhua (Flos Lonicerae), 3 g of Gancao (Radix Glycyrrhizae ) , 30 g of Banlangen (Radix Isatidis) and 10 g of Baijiangcan (Bombyx Batryticatus)

applicable to the patients with skin itching and dysphoria with the syndrome of stagnation of pathogenic factors in the defensive system.
