Sedative prescriptions refer to those mainly composed of sedatives with the effect of tranquilizing mind for the treatment of mental disorders.
By mental disorders is meant mental derangement with chief clinical manifestations as palpitation, insomnia, dysphoria and mania, the basic pathology of which may either be disturbed mentality by pathogenic heat or failure of blood to nourish the heart. Therefore, these prescriptions are accordingly subdivided into prescriptions with heavy sedatives and those with tonic sedatives.
Prescriptions with heavy sedatives are chiefly made up of tranquilizers of great weight, which bear the properties of tranquilizing mind, suppressing yang and purging away fire. They are indicated for restlessness of mind with chief pathologic changes as yang hyperactivity of the heart and liver as well as mental disturbance due to heat and fire. Prescriptions with tonic sedatives possess the effect of tranquilizing mind and nourishing yin and blood. They are suitable for irritability as a result of failure of blood to nourish the heart and liver due to deficiency.
Minerals, metals or shells in prescriptions with heavy sedatives are apt to impair stomach@. Prolonged administration is therefore inadvisable. Patients with usual weakness of the spleen and stomach should use some drugs which can regulate the spleen and stomach. Though in this group of prescriptions, Zhusha ( Cinnabaris ) is very effective in tranquilizing mind, it should not be taken in large dosage or for a long time to prevent mercurialism because its chief ingredient is mercuric sulfide.
Prescriptions with Heavy Sedatives
Zhusha Anshen Wan
( Cinnabar Sedative Pill)

Ingredients:
No. 1 Zhusha (Cinnabaris) 15 g;
No. 2 Huanglian (Rhizoma Coptidis) 18 g;
No. 3 Zhigancao (Radix Glycyrrhizae Praeparatae) 16g;
No. 4 Shengdihuang (Radix Rehmanniae ) 8 g;
No. 5 Danggui (Radix Angelicae Sinensis ) 8 g.
Administration: Grind the first ingredient in water or grind it into extremely fine powder; then mix it well with the fine powder of the rest; make the mixture into pills with honey and take 6 9 g each time with warm boiled water before going to bed. Or decoct the above except No. 1 in water with dosages modified proportionally on the basis of the original recipe, take with this decoction the first ingredient after it is ground in water.
Actions: Tranquilizing mind and purging away fire from the heart.
Clinical Application: This recipe is indicated for mental derangement due to hyperactivity of heart-fire and deficiency of yin and blood, marked by insomnia, dreaminess, severe palpitation with fright, vexation, irritability, reddened tongue, thready and rapid pulse. It is appliCable to palpitation, amnesia, and insomnia induced by heurosis, or to vague mind induced by mental depression and others, which belong to the flaring up of heart fire and deficiency of yin and blood. If complicated with phlegm-heat manifesting chest distress and greasy fur,add Quangualou (Fructus Trichosanthis ) and Zhuru ( Caulis Bambusae in Taeniam) to clear away heat and eliminate phlegm. In case of severe palpitation and insomnia, add Longgu (OsDraconis), Muli (Concha Ostreae) and Cishi (Magnetitum) to enhance the effect.
Elucidation: The syndrome results from hyperactivity of the heart-fire, causing impairment of yin blood and lack of nourishment in the heart. It should be treated with heavy sedatives and by purgating heart fire. The first ingredient is heavy and cold in nature and takes effect directly in the heart meridian. It acts as monarch drug with the effect of easing mind and purging fire from the heart.
The second ingredient functions as minister drug to help bring about the effect of the monarch drug. The combination of both achieves a remarkable effect of clearing away heat from the heart and relieving vexation. Ingredients No. 5 and No. 4 have the respective effect of nourishing blood and yin and are used as adjuvant drugs. Serving as guiding drug, Ingredient No. 3 can supplement qi and regulate the middle-energizer, prevent No. 1 from impairing the stomach, and mediate the properties of other drugs.
Cautions: This recipe is contraindicated for those with general debility or weakness of the spleen and stomach.
Cizhu Wan
(Medicated Leaven Pill)

Ingredients :
No. 1 Cishi (Magnetitum) 60 g;
No. 2 Zhusha (Cinnabaris) 30 g;
No.3 Sbenqu (Massa Dermentata Medicinalis) 120 g.
Administration: Grind the above drugs separately into extremely fine powder, then mix them with honey after being sieved to make boluses; take 6 g each time and once daily with warm boiled water.
Actions: Tranquilizing mind, suppressing yang and improving eyesight.
Clinical Application: This recipe is applicable to restlessness due to disharmony between the heart and kidney, marked by palpitation, insomnia, tinnitus, deafness, and blurred vision. It is also applicable to neurosis, epilepsy, hypertension, and pathologic changes of retina, optic nerve, vitreous body and lens as well as disturbance of aqueous humor circulation. If concomitant with dizziness, xenophthalmia and photophobia that are indicative of deficiency of liver and kidney yin, Liuwei Dihuang Wan may be taken simultaneously, or Liuwei Dihuang Tang for oral administration of the boluses. In case of epilepsy with excessive phlegm, Daotan Tang [omposed of Erchen Tang plus Tiannanxing ( Rhizoma Arisaematis ) and Zhishi (FructusAurantii Immaturus) ] can be used for oral administration of the boluses.
Elucidation: The syndrome is caused by deficiency of the kidney-yin, hyperactive heart-yang and incoordination between the heart and kidney. It should be treated by nourishing yin and suppressing yang, restoring coordination between the heart and kidney and tranquilizing mind. The first ingredient, which is pungent and cold in nature and takes effect in the kidney, is the monarch drug with the effect of supplementing yin, suppressing hyperactive Yang and easing mind. Used as minister drug, the second ingredient is sweet and cold in nature and takes effect in the heart, purging away fire from the heart and tranquilizing mind.
