Zhenren Yangzang Tang
(Zhenren Decoction for Nourishing the Zang Organs)
Source: Taiping Huimin Hejiju Fang (Benevolent Prescriptions from Taiping Pharmaceutical Bureau).
Ingredients:
No. 1 Renshen (Radix Ginseng) 18 g;
No. 2 Danggui (Radix Angelicae Sinensis) 18 g;
No. 3 Chaobaizhu (Rhizoma Atractylodis Macrocephalae Praeparatae ) 18 g;
No. 4 Roudoukou (Semen Myristicae) 15 g;
No. 5 Rougui (Cortex Cinnamomi) 24 g;
No. 6 Zhigancao (Radix Glycyrrhizae Praeparatae) 24g;
No. 7 Baishaoyao (Radix Paeoniae Alba) 48 g;
No. 8 Muxiang (Radix Aucklandiae) 42 g;
No. 9 Hezi (Fructus Chebulae) 36 g;
No. 10 Yingsuke (Pericarpium Papaveris) 108g.
Administration: Grind the above drugs into powder and decoct 6-12 g each time in water for oral application.
Actions: Astringing the intestines to stop diarrhea and warming the middle-energizer and reinforcing the spleen.
Clinical Application: This recipe is indicated for syndrome due to deficiency cold of the spleen and stomach and failure of the intestine to astringe, marked by protracted diarrhea or dysentery occurring irregularly day and night, abdominal pain with preference for warmth and pressure, lassitude, poor appetite, pale tongue with whitish fur, thready and slow pulse. It is applicable to longstanding cases of chronic or allergic colitis and others, which belong to weakness of the middle energizer. In case of severe aversion to cold with cold limbs, add Fuzi (Radix Aconiti Praeparatae ) and Ganjiang ( Rhizoma Zingiberis) to strengthen the effect of warming the spleen-yang. In case of marked tenesmic sensation in rectum or prolapse of rectum, add Huangqi (Radix Astragali seu Hedysari) and Shengma (Rhizoma Cimicifugae) to supplement qi and lift up the spleen-yang.
Elucidation: The syndrome is caused by impairment of the spleen-yang and failure of the intestine in astringent effect as the result of protracted diarrhea or dysentery. It should be treated by consolidating the intestines to relieve diarrhea and warming the middle energizer and invigorating the spleen. The last ingredient is monarch drug, in larger dosage and with the effect of consolidating the intestines and relieving diarrhea. Ingredients No. 9 and No. 4, help bring about the effect of the monarch drug on the one hand, and warm the middle-energizer on the other, acting as minister drugs. Ingredients No. 1 and No. 3 can invigorate qi and the spleen, No. 2 and No. 7 can nourish and regulate the flow of blood as well as relieve pain; No. 5 can warm the kidney and spleen, and No. 8 can regulate the flow of qi and activate the spleen, preventing qi stagnation possibly induced by tonic and astringent drugs.
These six ingredients together play the role of adjuvant drug. Combined with No. 1 and No. 3, ingredient No. 6 can achieve the effects of invigorating the middle-energizer and supplementing qi, and relieving spasm in combination with No. 7. Besides it also mediates the efficacy of other ingredients and serves as guiding drug.
Cautions: It is contraindicated for the early stage of diarrhea or dysentery, when the accumulation of damp heat is not yet removed.
Sishen Wan
(Pill of Four Miraculous Drugs)

Ingredients:
No. 1 Roudoukou (Semen Myristicae) 60 g;
No. 2 Buguzhi (Fructus Psoraleae) 120 g;
No. 3 Wuweizi (Fructus Schisandrae) 60 g;
No. 4 Wuzhuyu (Fructus Evodiae) 30 g.
Administration: Grind the above drugs into powder, which is then mixed with 240 g of Shengjiang (Rhizome Zingiberis Recens) and 50 pieces of Dazao (Fructus Ziziphi Jujubae) after they are boiled and the cores removed; make the mixture into pills and take 6 - 9 g each time and twice daily. Or otherwise make into decoction with dosages in proportion to the original recipe.
Actions: Warming and invigorating the spleen and kidney and astringing the intestine to stop diarrhea.
Clinical Application: This recipe is indicated for syndrome of diarrhea before dawn, marked by diarrhea at dawn, anorexia, or protracted diarrhea, abdominal pain relieved with warmth, soreness of waist and cold limbs, mental fatigue and lassitude, pale and large tongue with thin whitish fur, deep slow and feeble pulse. It is applicable to chronic or allergic colitis with morning diarrhea, which belong to yang deficiency of the spleen and kidney.
In case of severe soreness of waist and cold limbs, add Fuzi (Radix Aconiti Praeparatae ) and Rougui ( Cortex Cinnamomi) to enhance the effect of warming and activating kidney-yang. If concomitant with prolapse of rectum, add Huangqi (Radix Astragali seu Hedysari ) and Shengma (Rhizome Cimicifugae) to replenish qi and lifting up yang
Elucidation. The syndrome is caused by deficiency of kidney-yang, leading to failure of fire in warming earth. It should be treated by warming and invigorating the spleen and kidney to relieve diarrhea. As monarch drug, Ingredient No. 2 is capable of activating kidney-yang and warming the spleen to relieve diarrhea. The first ingredient acts as minister drug to warm the kidney and spleen. Ingredient No. 3 has an astringent and antidiarrheal action, and No. 4 bears the effect of warming the interior and dispelling cold. Shengjiang (Rhizome Zingiberis Recens) and Dazao (Fructus Ziziphi Jujubae) are effective in invigorating the spleen and regulating the stomach, and strengthen the effects of the monarch and minister drugs, serving as adjuvant drugs together with No. 3 and No. 4.
Both this recipe and Zhenren Yangzang Tang possess the action of consolidating the intestines and relieving diarrhea and are both used for treating protracted diarrhea and prolapse of rectum. But the former is good at warruing the kidney and spleen, and a typical recipe for morning diarrhea caused by deficiency of the kidney-yang and failure of fire to warm earth. While the latter focuses on warming the spleen-yang and is fit for protracted diarrhea due to impairment of the spleen and failure of the intestine in astringent action.
